Services tailored to FQHCs & Look-alikes (Community Health Centers)
TO ADVANCE THE HEALTH CENTER PROGRAM, TRANSFORMATIVE CONSULTING:
- Provides strategic consulting, strategic planning as well as business and service delivery planning
- Supports the development of new health centers, FQHC look-alikes and planning of site and service area expansions
- Conducts FQHC-tailored community health needs assessments, environmental scans and market analyses to aid health centers with strategic planning, business and growth planning, and preparation of funding applications
- Offers consulting and proposal writing related to the development of new urban and rural community health center sites - including coordinating/developing New Access Point (NAP) applications.
- Coordinates and develops health center Service Area Competition (SAC) applications to renew existing federal funding and remain compliant with federal program requirements.
- Develops capital-funding requests to improve, expand and build facilities
- Assists health centers to remain compliant with the Health Center Program Requirements
- Provides extensive consulting on health center scope of project and scope realignment, including the development of Change in Scope requests.
- Assists health center staff to analyze, address and respond to grant conditions.
